Descending thoracic aorta–abdominal aortic bypass and bilateral renal arterial blood circulation reconstruction are effective in atypical coarctation of the aorta with heart failure: a case report

Abstract Background There are a lot of reports the renal failure and heart due to coarctation aorta. However, there no case in which revascularization dramatically improved left ventricular function patients with progressive decline function. Herein, we present rare was by surgical treatment for dysfunction atypical Case presentation A 58-year-old man underwent axillary artery-bilateral femoral artery bypass at another hospital aorta Takayasu’s arteritis. Approximately 10 years later, he re-hospitalized failure, ejection fraction gradually decreased 28%. Computed tomography showed severe calcification stenosis same site from peripheral thoracic descending lower abdominal artery, aortography delayed bilateral blood flow. An increase plasma renin activity also observed. Despite administration multiple antihypertensive drugs, pressure control insufficient. We decided perform improve cardiac increased afterload activated activity. Descending aorta-abdominal arteries via great saphenous vein grafts were performed. Postoperative improved, dose drugs could be reduced. Plasma decreased, transthoracic echocardiography 1.5 later an improvement contractility 58%. Conclusion In flow, hypertension, decrease contractility, aortic recirculation can extremely effective.
